New Pharmacy Opening Package

Please review this package for information regarding a new pharmacy opening (package includes application).

PLEASE NOTE:  A licensed pharmacist cannot sign their name as a pharmacy manager on a new pharmacy application if they are currently a pharmacy manager at a licensed pharmacy.

Existing Community Pharmacy Licence Application

Change of Ownership Instructions:

Please return the following to the College office, within 7 days of the ownership change:

1) a completed Pharmacy Licence Application (above) by mail, fax, or in person (if the owner is not a member of the College, then an original ink signature of the owner is required).

2) a copy of the Articles of Incorporation from the new owner

3) the current licence with the appropriate sections on the back completed (original required)

4) the fee as stated on the bottom of the licence application

PLEASE NOTE:  The pharmacy licence number will not change, but Manitoba Health will issue a new Pharmacare Number. But until steps 1 – 4 are completed, College cannot notify Manitoba Health or issue a new licence.

Pharmacy Technician Final Check Application
 

The Pharmacy Technician Final Check Application was developed by the College for use in both community and hospital pharmacy. The application process requires pharmacy managers and staff to assess current dispensing processes to determine the changes required for a pharmacy technician to perform the final check safely.

Please refer to the Pharmacy Technician Final Check Information Sheet for additional information to help you build your Pharmacy Technician Final Check policies and procedures. Pharmacy managers must submit a copy of the proposed policies and procedures for Council approval using the Pharmacy Technician Final Check Application.

The College also developed a Pharmacy Technician Resource Guide and a Frequently Asked Questions document  that will help you develop your Final Medication Check policies for pharmacy technicians.

Pharmacy Technician-in-Training Application

Pharmacy Technician Renewal Application

For pharmacy technicians who wish to renew their listing as a pharmacy technician in the Province of Manitoba. The listing as a pharmacy technician expires on May 31st of each year.

Please complete the form and email to  accreditations@cphm.ca or fax (204) 237-3468. Please allow at least three weeks for a response to a completed accreditation submission.  

Program Provider Accreditation Package

As of October 17, 2016, College Council approved a new Program Provider Accreditation Package. This improved accreditation package is designed to safeguard the quality of professional development programs for Manitoba pharmacy professionals. 

Please review the accreditation package and submit all required materials by email to  accreditations@cphm.ca  or by fax to (204) 237-3468. Please allow at least three weeks for a response to a completed accreditation submission.
