Closing the Patient Information Gap: NIDR National Snapshot Spotlights Missing Critical Patient Information as a Factor in Medication Incidents  

In July 2023, The National Incident Data Repository for Community Pharmacies (NIDR) published its National Snapshot to share learning about the types of medication incidents that have been reported by community pharmacies across Canada. This NIDR analysis focuses on “critical missing information” as a contributing factor in 315 medication incidents including missing information during the processes of prescription order entry, pharmacist clinical check, vaccine administration, and/or prescription delivery.  

The National Snapshot is a learning tool that also offers simple, actionable recommendations to help you reduce the chances of patient harm in your pharmacy.  

This report, and other resources published by ISMP Canada, would not be possible without the contributions of pharmacy professionals like you. Your commitment to sharing data with the NIDR ensures that health care professionals across Canada learn from medication incident and near-miss events. We all benefit from raised awareness of potential risks and simple improvements that can reduce the chances of patient harm in community pharmacy practice.

Reminder on How to Access CPSM’s Methadone and Suboxone Prescriber List

This serves as a reminder that the College of Pharmacists of Manitoba (CPhM) has collaborated with the College of Physicians and Surgeons of Manitoba (CPSM) to introduce an improved process for accessing the CPSM Methadone and Suboxone Prescriber List. 

To access the list, CPSM has developed a dedicated prescriber portal exclusively for pharmacists. To gain access, please contact your pharmacy manager for the private username and password. 

Once logged in, you’ll have the convenience of conducting real-time searches on prescribers, enabling you to make well-informed decisions promptly. 

Key Points to Note: 

  • For optimal CPSM Portal performance, please ensure you use a modern browser. Older versions and Internet Explorer may cause website loading issues. 
  • The CPSM Pharmacist Portal will automatically log you out after a period of inactivity for security reasons. If this occurs, simply re-login to continue your work. 
  • Remember, this portal is designated solely for pharmacist use.
